Under-eye hollows and shadows

Tear trough filler in Hale, Altrincham

Tear trough filler can soften under-eye hollows and the shadows they create, helping your eyes look fresher, brighter and less tired.

Concealer covers the colour, but that little dip beneath your eyes still catches the light. Perhaps it's become deeper over time. Or you've always had under-eye hollows and would quite like them to stop making you look tired.

Tear trough filler restores volume beneath the eyes, smoothing the hollow where the lower eyelid meets the cheek. As the dip becomes less obvious, so can its shadow.

Dr Alex will examine your under-eye area and explain the improvement he thinks is achievable. The aim is for you to look in the mirror and see fresher-looking eyes, with less of that tired appearance underneath.

Does this sound familiar?

Your eyes look more tired than you'd like

Under-eye hollows can be a long-standing feature or something you've noticed becoming more obvious recently.

There's a groove beneath your eyes

You can see a dip running from the inner corner towards your cheek. It creates a shadow that can make the under-eye area look darker, especially in certain lighting.

Concealer isn't quite doing the job

You can cover some of the darkness, but the shape underneath is still there. You're wondering whether treating the hollow could make more difference than another layer of makeup.

You've always had these shadows

This isn't necessarily a new change. Your under-eyes may have looked hollow for years, and you'd like to know what treatment could do for the face you have now.

The details of under-eye filler

Tear trough filler treats a hollow beneath the eye. At consultation, Dr Alex will check whether that hollow is the main cause of the shadow you see.

A fresher eye area

Less shadow, more of your eyes

Softening the hollow can make the transition from your lower eyelid to your cheek look smoother, so your eyes look less tired.

You may notice that the groove is less visible from the front, or that the shadow beneath it no longer looks as deep. The change comes from improving the contour, rather than adding a lighter colour to the skin.

If both hollowing and pigmentation contribute to your dark circles, filler may improve the shadow while some skin colour remains. Dr Alex will explain which part he expects to change, so you have a realistic idea of the result.

The aim is to soften the dip without creating a ridge or unnecessary bulk beneath the eye. That can mean leaving a little natural contour rather than trying to make the area completely flat.

Would filler help?

What's behind the darkness under your eyes?

Hollows, skin colour and puffiness can look similar at a glance, but they don't need the same treatment.

The hollow is creating a shadow
Filler can be helpful when a dip below the eye is a main reason the area looks dark or sunken. Dr Alex will assess the contour and skin before recommending treatment.
The cheek below has lost support
Sometimes changes in the upper cheek contribute to the under-eye appearance. Dr Alex may recommend addressing that support first, then assessing whether the tear trough itself still needs filler.
Previous treatment needs checking
If you've already had under-eye filler, tell Dr Alex - even if it was years ago. Remaining product or persistent puffiness can change the recommendation, and adding more isn't always the answer.

Dark circles and eye bags

Filler doesn't treat every cause

Filler doesn't remove pigment from the skin or remove an eye bag. If swelling, prominent bags or loose skin are the main problem, filling the area may give little benefit or make it look puffier.

Dr Alex will explain whether skin treatment, another approach or a surgical opinion would be more useful. A consultation can help you identify the cause without committing you to filler.

How it works

Softening the dip that casts the shadow

A hyaluronic acid gel adds volume beneath the hollow, reducing the difference in height between the lower eyelid and the cheek.

A hollow can look dark because of the way light falls across it. Making that dip shallower can reduce its shadow, which is why the eye area may look brighter after treatment.

This is different from treating brown pigmentation or the colour of blood vessels visible through thin skin. Filler changes the contour. It isn't a skin-lightening treatment, and it doesn't replace treatments aimed at skin quality.

Your appointment

Start with what you see beneath your eyes

Show Dr Alex the hollow or shadow that bothers you. He can examine you in person and explain which changes treatment is likely to make.

At consultation

Assess the cause

Dr Alex will assess the hollows, skin, any puffiness and the cheek support below. Tell him about previous filler, eye-area procedures and swelling you've noticed, as well as your medical history and medicines.
Before treatment

Agree on the likely improvement

You'll discuss what filler could change, what it is unlikely to change, the proposed product, risks and cost. If another approach looks more suitable, he'll explain why.
If you go ahead

Treat the hollow

Dr Alex places the filler using the technique suited to the area, which may involve a needle or a fine blunt-tipped cannula. The HA filler he uses contains lidocaine to help with comfort.
After your appointment

Let the result settle

You'll receive aftercare advice and guidance on when to assess the contour. Contact the clinic about persistent puffiness or any unexpected change. Additional filler in the same area is not considered before at least a month, and may not be needed.

Why choose Dr Alex?

An experienced filler doctor for your under-eyes

Dr Alex teaches doctors, dentists and nurses how to carry out dermal filler treatments. His experience spans several filler ranges, including his current role as a trainer for Evolus's Estyme range.

For under-eyes, choosing whether to use filler is as important as carrying out the injections. He'll examine the hollow, the skin and the cheek beneath it, then explain what is contributing to the tired appearance you want to change.

You can ask him directly how much of the shadow he expects to improve. If the main problem is puffiness or skin quality, he'll discuss options aimed at that concern rather than simply adding volume.

Side effects and suitability

What should you know about under-eye filler?

Bruising, tenderness and swelling can occur after treatment. Because the skin here is thin, small irregularities may be more visible than elsewhere on the face.

Possible side effects

Other possible problems include persistent puffiness, unevenness, visible filler or a blue-grey tint, infection and delayed inflammation. Contact Dr Alex if a change isn't settling or develops later.

Filler can also block a blood vessel. This is uncommon but can cause tissue damage, and very rare complications include permanent loss of sight. Neither a cannula nor a particular technique makes treatment risk-free.

Symptoms that need immediate attention

Seek urgent help for increasing or unusual pain, pale or mottled skin, or a dusky colour around the treated area. Contact SONÜ immediately and seek urgent medical care if the clinic is unavailable. For any change in vision, sudden weakness or difficulty speaking, call 999 and say you've had filler.

When filler may be unsuitable

Active infection or inflamed skin near the eyes needs assessment before treatment. Tell Dr Alex about pregnancy or breastfeeding, allergies, medicines and previous filler reactions. Recurrent under-eye swelling, significant bags or existing filler problems may make another approach more appropriate.

Have the questions answered before deciding whether treatment is right for you.

A few more questions

Questions about tear trough filler

Including dark circles, eye bags and how filler compares with skin treatments.

Is tear trough filler the same as under-eye filler?
Usually, yes. Tear trough refers to the groove beneath the lower eyelid, particularly towards the inner corner. Under-eye filler is the more everyday name. The consultation establishes the exact area that needs treatment rather than relying on the booking label.
Will it get rid of my dark circles?
It can reduce darkness caused by a hollow casting a shadow. Brown pigmentation, visible vessels through thin skin and other causes of darkness won't necessarily improve with filler. Dr Alex will examine the area and explain which part of your dark circles he expects it to help.
Does it remove eye bags?
No. Filler adds volume; it doesn't remove a bag. It may sometimes soften a neighbouring hollow, but that doesn't make every eye bag suitable for filler. If bags, puffiness or loose skin dominate, Dr Alex will discuss a different approach.
Would polynucleotides be better?
They address different concerns. Filler replaces volume in a hollow. Polynucleotides are used for skin quality, including crepey skin and poor elasticity; they aren't a replacement for filler when missing volume is the main problem. You may benefit from one, the other or a combination, depending on the assessment.
Do I need cheek filler first?
Sometimes, if lost support in the upper cheek is contributing to the under-eye hollow. Treating that area first may reduce how much direct tear trough treatment is needed. It isn't a rule for everyone, and Dr Alex will explain the reason if he recommends it for you.
How long does it last?
Dr Alex usually advises 12-18 months for visible HA filler results, but under-eye filler can remain for longer. He'll assess the contour and any existing product before recommending more, rather than assume you need a yearly top-up.
Will the injections hurt?
You may feel a brief sting or pressure. The filler contains lidocaine to help reduce discomfort, and Dr Alex can discuss additional numbing if needed. Tell him if you've found previous filler treatment uncomfortable.
How much time should I leave before an event?
Avoid booking it immediately before an important event. Bruising and puffiness can be visible beneath the eyes, and settling varies. Tell Dr Alex the date you're working towards so he can advise on a sensible gap for your treatment and any review.
Can under-eye filler be dissolved?
Hyaluronic acid filler can be treated with an enzyme called hyaluronidase. Dissolving needs a separate assessment and carries risks, including an allergic reaction; it isn't a guaranteed instant reset. Dr Alex will discuss whether it is appropriate if existing filler is causing a problem.
What if I've had tear trough filler elsewhere?
Tell Dr Alex when it was done, what was used if you know, and whether you've had swelling or lumps since. Old filler may still be present. He'll assess it before recommending anything further, including whether it is better to leave the area alone.
