With Dr Alex, a filler educator who teaches doctors, dentists and nurses and has helped introduce filler ranges to the UK.
What has changed?
The jawline that used to look straighter
A small break in the outline can draw more attention than you'd expect, particularly beside the chin.
The practical details
Jowl filler at a glance
Filler is most useful when improving the neighbouring contour would make an early jowl less noticeable.
The result
A straighter jawline can look less jowly
Blending the dip next to a jowl can reduce the contrast between the hollow and the bulge, making the outline look more even.
The effect comes from changing the contour around the jowl. Where a hollow breaks the jawline, restoring support can create a smoother transition towards the chin and soften the shadow in that dip.
You may want to regain the definition you remember rather than create a sharper or wider jaw than you've ever had. Dr Alex will discuss that with you, including any preference for a more masculine or feminine shape. The recommendation should suit your features as a whole.

Is filler the right option?
A small jowl and a hollow are different problems
Dr Alex looks at how much of the appearance comes from missing support and how much comes from loose or heavy tissue.
Camouflage, not removal
Filler does not lift away a heavy jowl
It doesn't remove excess skin or fat. Adding gel to a lower face that is already heavy can make it bulkier without giving the definition you want. If the jowl itself is the main problem and there is little useful contour to restore, more filler may achieve very little.
How it works
Blending the dip beside the jowl
Jowl filler usually improves the outline by restoring support next to the jowl, rather than injecting more volume into the bulge itself.
Age-related changes don't happen evenly. As support around the jaw changes and tissue descends, a hollow can develop beside the chin while the neighbouring jowl becomes more noticeable. The difference in height creates an uneven edge and a stronger shadow.
Hyaluronic acid gel can support the hollow so the jawline flows more smoothly through that area. The jowl is still there, but the dip that drew attention to it is less obvious.
Dr Alex also assesses chin projection and the rest of the jawline. Sometimes treating an adjacent area would improve the outline more than repeatedly adding filler beside the jowl.

Why Dr Alex
Why choose Dr Alex for jowl filler?
Knowing where filler will help is particularly useful around jowls. A dip may benefit from support; a heavy fold may simply look heavier if more is added. Dr Alex will examine both before recommending a treatment.
He teaches doctors, dentists and nurses how to use dermal fillers, including in his current role as a trainer for Evolus's Estyme range. He has also helped launch Estyme and Aliaxin LV in the UK.
Your preferred result guides the assessment. If you want to restore a smoother jawline without a broader or more angular lower face, say so. He'll explain what can be achieved with the contours you have.

Side effects and suitability
What should you know about jowl filler?
The treated area can be tender, bruised or swollen for a while after the injections.
Possible side effects
Other possible problems include unevenness, lumps, infection, allergic reactions or inflammation that develops later. Adding too much can also produce a bulky-looking contour.
Filler can uncommonly block a blood vessel, causing skin damage. Very rarely, vascular complications can affect sight or cause a stroke. Appropriate technique reduces avoidable risk but cannot make filler risk-free.
When to seek urgent help
Unusual or increasing pain, or skin that becomes pale, grey, dusky or mottled, needs urgent assessment. Contact SONÜ immediately, or seek urgent care if the clinic is unavailable. Call 999 for any change in vision, sudden weakness or trouble speaking, and mention the recent filler.
Before having treatment
Discuss your medical history, medicines, allergies, pregnancy or breastfeeding, and previous filler reactions with Dr Alex. Active infection or inflamed skin in the treatment area may mean postponing injections.
The possible benefits and risks are explained before you choose treatment.
